<description>There are a huge number of such lapses one can encounter. 
1. Keeping of files containing import information in places without even basic fire-alarms much less fire-drills and awareness
2. Absence of security personnel at vital and public places

etc.,

All of them boil down to these 3 things:

1. Apathy of the common citizen to anything that doesnt directly impact her/his own well-being.

2. Lack of awareness about security - in all walks of life

3. Governmental apathy.</description>
